Funded place agreement. Every funded place has been paid for to give a child the opportunity to take part. Please only reserve sessions that you genuinely intend to attend. We completely understand that children become poorly and plans can change. If you cannot attend, please contact us or cancel your place as soon as possible so that we have an opportunity to give the place to another child. Please confirm: (copy) *

I understand that applying does not guarantee a funded place.
I understand that I may not receive every session I have requested.
I will only request sessions that I genuinely intend for my child to attend.
If my child cannot attend, I will let 5 On It Foundation know as soon as possible.
I understand that if my child does not attend a funded session and I have not cancelled or contacted 5 On It Foundation, any remaining funded places allocated to my child may be cancelled and offered to another family.

How we will use your information

5 On It Foundation takes the privacy and safety of children and families seriously.

The information you provide through this form will be used to:

assess and administer your application for a funded Nutri Hoops or Nutri Netters place;
contact you about your application and allocated sessions;
manage attendance and funded places; and
ensure our team has appropriate information to support your child safely during their sessions.

Some information you choose to provide may be sensitive, including information about your child’s health or additional needs and information about your family circumstances.

Please only provide information that is relevant to your application or that our team needs to know to support your child.

Who can see your information?

Your information will be treated sensitively and confidentially.

Access will be limited to appropriate members of the 5 On It Foundation team and, where necessary, relevant coaches who need information to safely support your child.

We will not provide HORIBA UK with your child’s medical information or details of your family’s personal or financial circumstances.

We may provide HORIBA UK with anonymised information about the programme, such as the number of funded sessions delivered or children supported, to demonstrate the impact of their funding.

How long will we keep your information?

We will only keep personal information for as long as reasonably necessary for the purpose for which it was collected and in line with our data protection responsibilities.

You can contact 5 On It Foundation if you would like to ask what personal information we hold about you or your child, request that inaccurate information is corrected, or ask for information to be deleted where applicable.

We have a limited number of funded places available and unfortunately cannot accept every application. Please do not attend a session until you have received confirmation from us that your child’s funded place has been approved.
